How to create a Jotform Survey.

  1. Start at jotform.com

  2. Create an account (if you already have an account, skip this step)

  3. At the top left or center of the page, click the "Create Form" button.

  4. Choose an option to start from scratch or a template depending on your needs.

    1. Note: Jotform has one of the most templates (over 10,000+)!

  5. The page is then divided into three tabs: Build, Settings and Publish.

    1. In the Publish Tab, add collaborators by clicking on the "Share Options" Button in the middle of the page.

    2. Build Tab

      1. Add a logo to your form at the top of the page.

      2. To control the looks of your form, click on the paint icon that brings up the Form Designer.

      3. Click on the "Add Form Element +" on the left top of the page.

      4. Add questions by dragging in form elements of either Basic, Payment or Widget Types.

      5. Each question/form element has properties, such as alignment and placeholder. You can set these properties by clicking on the tool icon on the right side of each form element.

    3. Settings Tab

      1. Set a title for your form.

      2. Choose when to enable your form.

    4. Publish Tab

      1. Copy the link to share the survey.

      2. Explore various embedding options using the bar on the lefthand side of the page.
