How to create a Jotform Survey.
Start at jotform.com
Create an account (if you already have an account, skip this step)
At the top left or center of the page, click the "Create Form" button.
Choose an option to start from scratch or a template depending on your needs.
Note: Jotform has one of the most templates (over 10,000+)!
The page is then divided into three tabs: Build, Settings and Publish.
In the Publish Tab, add collaborators by clicking on the "Share Options" Button in the middle of the page.
Build Tab
Add a logo to your form at the top of the page.
To control the looks of your form, click on the paint icon that brings up the Form Designer.
Click on the "Add Form Element +" on the left top of the page.
Add questions by dragging in form elements of either Basic, Payment or Widget Types.
Each question/form element has properties, such as alignment and placeholder. You can set these properties by clicking on the tool icon on the right side of each form element.

Set a title for your form.
Choose when to enable your form.
Publish Tab
Copy the link to share the survey.
Explore various embedding options using the bar on the lefthand side of the page.

Jotform's interface may take a bit of time for users to get familiar especially when with working with form fields as individual settings for them are a bit hidden and thus come with a slight learning curve.

Despite having a 24/7 support chat, Jotform's average response wait time is approximately 6 hours, which is very long, especially for customers dealing with urgent matters.
Jotform offers five different tiers to their platform - Starter, Bronze, Silver, Gold, and Enterprise.
The Starter tier is free, but it is limited to 5 forms, 100 responses, 100MB space, 1,000 monthly form views, 500 submission storage, and 10 monthly payment submissions.
The Bronze tier is $29/month, and it comes with 25 forms, 1,000 responses, 10GB space, 10,000 monthly form views, unlimited submission storage, and 100 monthly payment submissions.
The Silver tier is $39/month, and it comes with 100 forms, 10,000 responses, 100GB space, unlimited monthly form views, unlimited submission storage, and 1,000 monthly payment submissions.
